“Introduction to Root Cause Analysis” Abstract
In this introductory course you will learn about some of the popular methods for root cause analysis. The methods that will be discussed in this class are “The 5 Whys”, failure mode and effects analysis (FMEA), Pareto Analysis, fault tree analysis, and the Ishikawa diagram (Fishbone diagram or cause and effect). During the class presentation there will be exercises for each of the methods. You will learn to identify the problem, determine the significance of the problem, identify the causes (conditions or actions) immediately preceding and surrounding the
Problem, and identify the reasons why the causes in the preceding step existed. At the end of the class you will have a basic understanding of the methods and how they apply to problem solving.
